As parent/guardian of the above player, I certify that he/she is in excellent health and has no physical, mental or emotional problems that maybe likely to prevent participation in strenuous physical play at lacrosse clinics and camps. I agree to hold harmless CK1 Lacrosse LLC and it's employees and hereby release them from any liability on account of injury sustained during such participation and certify that he/she is covered by medical insurance which will reimburse CK1 Lacrosse LLC for expenses incurred by them, their agents and employees on account of medical procedures ordered at their discretion and also indemnify them from any expenses not reimbursed by such insurance. I give consent for my child/player to be photographed, videotaped or filmed while participating in the clinic/camp/ and or resulting images to be used by CK1 Lacrosse LLC for educational and promotional purposes.
I am aware there are risks to me and my child of exposure to directly or indirectly arising out of, contributed to, by, or resulting from an outbreak of any and all communicable disease, including but not limited to, the virus “severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2)”, which is responsible for Coronavirus Disease (COVID-19) and/or any mutation or variation thereof.
As a Parent or guardian of the participant I HEREBY RELEASE, INDEMNIFY, AND HOLD HARMLESS CK1 Lacrosse LLC, its employees, and, if applicable, owners and lessors of premises used to conduct the event, from any and all claims, demands, losses, and liability arising out of or related to any ILLNESS, INJURY, DISABILITY OR DEATH I and/or my child may suffer, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE, to the fullest extent permitted by law.
As a Parent of an athlete it is important to recognize the signs, symptoms, and behaviors of concussions.
By checking below you are stating that you understand the importance of recognizing and responding to the signs, symptoms, and behaviors of a concussion or head injury. Any athlete suspected of suffering a concussion should be removed from the program immediately. No athlete may
return to activity after an apparent head injury or concussion, regardless of how mild it seems or how quickly symptoms clear, without medical clearance. At CK Lacrosse, concussions are taken very seriously.
I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.
